I want to say thanks and shout out Natalie from Activities for always having a great spirit when in the presence of the patients having genuine interest in what they enjoy and puts a lot of thought and effort in making sure they have a great stay at this rehabilitation center I have my dad at. Out of all the centers that he’s been to within the past year he says he really enjoys this one, shout out to the people like Natalie and other amazing staff like Lidia and Daravy because they’ve treated him like a person and not just a number, which means the world to him. Thank you to the Community Care Rehabilitation staff for always being amazing human beings that show true care for what they do, I want to shout these people out for giving great care for not only my own father but I’ve seen them put smiles on many other residents faces, people who really need the love and care are made sure they receive it from those who come in rain or shine with a great attitude. Shout out to the QA Angelica who always has a smile on her face and making sure things are in shape around the facility, Annikah in physical therapy for giving so many people hope as they strive to continue their progress, it’s really been giving my father faith and he looks forward to working to get stronger with his walking, Also shout out to Ryan in the kitchen, CNA Carol, Luiz, Lucille,and Dalidai and many more, as well as Lidia and Daravy for also taking the time out of their day to make sure the patients are enjoying their stay. Celebration of Social workers Month

What an honor and privilege it is to celebrate the contribution of excellent social workers to the wellness of the CCRC community. March is known as National Social worker month, to the end we salute the distinguish, noteworthy service of Martha Huerta a resident social work at this facility. Huerta has worked in many capacities in social services since 2009. She began her work in nursing homes in 2000 as a Certified Nursing Assistant. She expanded her training as a Restorative Nursing Assistant, central supplies and became an assistant to Director of Staff Development broading her scope of understanding nursing operations. For the past 8 years she has serviced CCRC utilizing her broad skills. The scope of her services include, appointment scheduling, transporting, care plan meetings and making recommendations such as psychosocial needs of residents. Martha is known for equality of services and treats every person with respect and dignity. Brilliance and grace caring with competency is her signature which is most noteworthy. The challenge and responsibilities of those who work with seniors and individuals with disabilities who require long term care is an awesome challenge because the social aspect of addressing the multiple needs of patients and residents Is ominous. The services required by those who are called social workers takes commitment and dedication to improvement of the individual by assuring that medical and social needs are met with the goal to improve the lifestyle of the individual/resident. In addition to assisting that lifestyle and basic needs are met with excellence is the responsibility of advocating for patients/ residents. Needless to say the responsibilities of asocial worker is magnificent! In the regard we applaud the services of Martha Huerta appointed Social Worker of station 2 at CCRC. I just want to tell a little bit about my experience at CCRC while rehabilitating my leg. I want to give a shout out to Kimberly my amazing and beautiful nurse who really took care of all my needs and was always there to give a little conversation when I needed it and to Bryza (my other Nurse) whose smile was like a ray of sunshine in the morning. Both of these women had an amazing bedside manner and always made sure I had what I needed. Now a shout out to Lordnense (LuLu) i know I spelled her name wrong and I apologize for that; She was my case worker and she takes care of her patients to the point of getting me to my doctor’s appointment even if the cost of transportation had to come out of her own pocket. What a great staff all the way around including all the CNA’s who work way too hard and the OC & PT staff that got me walking, Thanks everyone.

For most of the nurse are very attentive and caring. Quite a few amazing ones, but there is one nurse so special her name is Kim and she is great always sweet and kind with a beautiful smile on her face, between her and michelle my stay was comfortable and home like. I have had a great experience here. Thank you CCRC, For helping me through this difficult time. I would definitely recommend this facility!

My experience at CCRC was a positive one. If I have to choose a skilled nursing home again I would choose CCRC. I would like to give a shout out to Susan, Manny and Sandra the night shift CNA. Also, Teresa, Haylie , Cynthia, and Mary CNA’s morning shift. Elbert and Regine in physical therapy. Leann, Justine, Kim, and Keira. Wound nurse Brian and Trish, and a big shout out to my case manager, Lourdes and Olga and Daravy. There are so many more that are great and I apologize that I can’t remember names but I remember faces. I thank you for who you are and what you do.

My father just left the facility, the nurses are AMAZING!!!!!! Such lovely people- much praise to my dad’s staff, Cinthia, Sonia, ester, Lourdes and Marietta! Forgive me if I missed you…. You were ALL great!!!!!!

What a wonderful experience for my husband while in rehab this past week. Everyone (and I do mean everyone) was attentive, friendly and on top of all his medical & personal needs. Upon discharge, they had made follow-up appointments with his Primary Care Physician & specialist. A big thank you to his Case Manager, Lourdes, and to every nurse & staff member for the best service with a smile. We walked in feeling unsure & left knowing if either of us needed this service in the future, Commuinty Care would be our first choice.

The people were good in there had their very nice and for being a riverside. What I’ve proposed to do to California. It wasn’t a bad placould be different but it was okay.

We did it again! Valentine’s Day Dinner, February 14, 2024, CCRC celebrated the day of love with beautiful classic romantic music and an excellent dinner of Gourmet pork ribs, buttered green beans, smacken macaroni and cheese with wheat dinner rolls and cheesecake dessert. The young chef at the helm was Sir Garret Manahan, assistant Dietary Director. Elizabeth Bustillos, Activities Director, serenaded the residents with questions in exchange for midnight sweet treats. The night ended with residents singing and chatting with sweet songs in delightful harmony. Applause is due for Director Elizabeth and her magnificent staff of excellent workers. Hats Off! Another great success!

In recognition of Black History Month, a period dedicated to honoring the extensive and multifaceted contributions of African Americans over more than four centuries, our facility has engaged in a series of commemorative activities. This initiative, aimed at celebrating the rich heritage and significant achievements of African Americans both within the United States and globally, was spearheaded by Charmina Jackson, LVN, who is endearingly referred to as “CJ”. With the assistance of Carol Aimua, CNA, and under the auspices of Darvy Perez, Director of Guest Relations, the walls of our establishment were adorned with posters that eloquently highlight the distinguished legacy of African American individuals.

Charmina Jackson, known for her daily spiritual encouragements on Station 2, has extended her celebration of Black History Month throughout the entire facility, ensuring that every corner reflects the pride and respect we hold for this important observance. As we approach the conclusion of February, we invite all members of our community to take a moment to reflect on the significant contributions African Americans have made.

We extend our heartfelt gratitude to Nurse Charmina for her exceptional dedication, not only as a skilled LVN but also as a beacon of moral integrity and dignity. Our appreciation also goes to Darvy for her unwavering support of both residents and staff, further elevating the standard of excellence within our community.

I’m not one to normally write reviews, but I’m very satisfied with this center. The way they have treated and taken care of my father Marlon is beyond amazing. I appreciate every single one of you guys. I want to give a huge shout-out to Daravy, Lidia, Ryan, and Natalie from activities and Annikah, John, Randy, Cindy, and Carol from rehab. Hats off to every single one of you, your job is not easy and you don’t get enough praise and recognition for what you do. The way you guys have taken care of my father shows me how much you care for your patients. Once again thank you so much from the bottom of my heart.

Day 1 at CCRC..I didn’t know what to expect. But it was one constant. Daravy Perez. She stepped up. When people don’t feel recognized for their efforts, they are less likely to put in the effort. But not Daravy! I’m amazed by how she maintain her performance while taking on even more work. Your efforts are deeply appreciated. Amelia’s work and personality stands out to. Always going! Much of the staff at CCRC are helpful and well liked. The guys ( therapists ) at the gym are great. Couldn’t have asked for a better team! Thanks to Ryan and the Kitchen crew. He looks out for everyone. Thanks Esmeralda, was always nice.

One of the best new additions to CCRC is the presence of Daravy Perez, Director of Resident Relations. She is creative and enthusiastic in the execution of her professional responsibilities. Residents are encouraged to know that her door is open to listen and assist in any possible. With “Perez” at the helm keep your eyes and ears open for bold and positive developments.

My wife had a wonderful stay here at CCRC, thanks to Daravy Perez. She is the best thing that has happened to this business. I am so grateful for her and what she has provided.

My dad came for rehabilitation for broken spine, hip, and femur. He said he was treated better than Riverside community hospital. This facility has two sides. They have a rehab side and a long term care side. We were in the rehab side. There was no foul smells. My dad didn’t complain about anything. He said the food was actually good!

There are two people per room and they have access to a patio from their room.

I want to say I have made good friends with the volunteers in activities especially Doug who has given me a lot of inspiration, and strength to continue to fight for my health. This also includes Liz the activities director and her employees Natalie and Bailey.

Daravy Perez was very professional, outstanding, and great with my mother’s care and family while she was under their care. Thank you so much to all the staff. Great care.

My mother in law recently had a stroke and was placed in this facility. It is clean and easy to communicate with the staff. The quality of care depends on the family members. You as the family have to be on top of your loved ones care and let them know you are involved in every aspect of their care. Call frequently and visit as much as possible so that they know you are paying attention to your family members care. Get to know the staff and caseworker, make sure you are on top of any services your loved one is entitled to and the days they receive them. This has been a difficult transition for my mother in law and our family. We are still adjusting and have found that communication is key at this facility for the well being of your loved one.

I would really like to thank all CNA’s, all the Physical Therapy staff and everybody in the building even Amelia the one that does housekeeping. I mean everybody in that place took great care of me and this is the second time I’ve been there and if I had to go back again I would go back. Everybody gave me a good welcome and took care of me, even the lady that does the Bingo and stuff.  I really appreciate you all for taking care of me and thinking about me when I needed help and the good care I got from there. Thank you very much. I recommend you to everybody. It’s the best place ever.

I just want to say that this is an amazing facility that my dad is in a spot that focuses on his care and recovery. I also want to give a shout-out to Joslynn Perez at the front desk and Daravy Perez in the Business office who has always gone above and beyond for my dad since we’ve been here showing amazing customer service and genuine care.

This is the place to be the people here are fantastic and this is a matter, what their job is or what their title is they do everything for everyone no matter what the title is, I think it could be the President that could be the democracy of anybody. This review is strictly for Darvey who works here! She’s Incredible and I cannot thank her enough for all she has done for me and my family. I had a friend and loved one stay at this facility, who recently passed away. Throughout the time there Darvey was always friendly, smiled, and respectful. She always looks as thou she loves her job and helping people. She is truly awesome at her job and I CANNOT THANK HER enough. The day my loved one passed she came and sat with me until she was picked up and was warm and caring. I just love her! I would recommend this facility there staff is great and help and care for their patients. The facility is clean and the staff are very friendly.

My husband just returned home after 4 months here. I can’t say enough about how wonderful this place is. The level of care is outstanding. There is a large team of professional caregivers to see to a patient’s every need.

I received friendly help from everyone with my questions and concerns. Everyone was so kind, from the nursing staff, physical therapists, and business and social services offices to the welcome at the front desk. The building is clean and well-maintained. Also, he was very happy with the food.

I stayed at the Center for 3 weeks. From the moment I arrived everyone was extremely nice and helpful. Daravy Perez gave out welcoming baskets, filled with wonder goodies and when I left She gave me flowers and a Chocolate Cake. She always made me feel welcomed, along with many others in charged. The nurses were wonderful, everyone in Physical Therapy were professional and warm. they help strengthen my legs and arms. I defiantly got stronger. Dr. Matar was very knowledgeable and although I was looking forward to getting home, they all made it very comfortable. I would recommend this facility to anyone that needs help. I will miss all the wonderful people that worked there, I wish them all the best and thank them for all they did for me.

All aspects of care at this facility are very good. I have a loved one here at this time and I have been watching the staff very carefully. The nurses are very attentive and well trained. Most of them are pretty friendly. I want to give a special shout out to DARAVY. She is the resident relations person. She checked in every day, several times a day to make sure everyone was comfortable and had water and anything we needed. I also want to give a special shout out to the hospice nurses JENNY and HAYDEE. They are very professional, knowledgeable and got things done very quickly when they noticed a change and my love ones condition. Lastly, I want to give the most special shout out to AMELIA. She she is a housekeeper and extremely sweet and helpful getting vases for flowers and checking in on my loved one. I would not hesitate to bring a loved one to this facility for hospice care or any kind of rehabilitation. The facility is clean and well stocked with supplies.

My brother has been in the care there at Community Care and Rehabilitation for the past 9 months. His case has been complicated and the entire staff including nurses, administrators, Social Workers, and those in the Business Office have been such a blessing to both my brother and myself. Navigating the best care for a family member to include ensuring they are covered under Medi-Cal, getting the right treatments, and understanding how each part of the systems work together is complicated and overwhelming. Janet, Maricela and the staff there at Community Care and Rehabilitation Riverside have gone above and beyond to help me advocate for my brothers well being.

COVID has placed additional stressors on the healthcare professionals and staff. Patients are also feeling the impact. Even through this period,
they have continued to provide exceptional care under the most trying circumstances. My Brother Bruce did test positive and was immediately moved into into a COVID ward with other positive COVID patients. This was a scary time for both my brother and I. I emailed Janet, the Head Social Worker outside business hours and she replied immediately. This attention to detail and professionalism during a crisis is impressive. My brother is doing well and I know he is in good hands there.

I’m so grateful for them to take my dad who’s a medi-cal patient. I worked with Carlos and Cindy, and they were a great help. They were all so nice to me and my sister, one time i got so hungry because I had to wait for their paperwork, I told them that I needed to step out because I’m diabetic and I needed to eat. Cindy was nice enough to offer me a sandwich. I mean who does that right? Anyway, the staff are very nice as well. I’m so glad that they also call me on step by step basis with my dad, when he goes through admissions by testing him, if he’s put to regular rooms, stuff like that. Its just a peace of mind. I was a caregiver to my dad, and I’m finally at ease that he’s in good hands.

My 92 year old Grandfather was transferred here from Parkview after he broke his hip. The attention and care he received here was amazing. My Grandfather can be stubborn at times but the staff was so patient with him. From the coordinator to the nurses to the cleaning crew everyone did there part to assure my Grandpa received the best care for the 3 weeks he was there. 2 months later my Grandfather is on his way to a full recovery.

Highly recommend this facility! The staff is great and very friendly. The residents are greatly taken care of and the nursing is very attentive to their needs. Administration staff is extremely helpful and resourceful. Love coming here to visit my loved one.

My dad has been her for about 6 weeks after a miserable stay at another facility. The staff are amazing. They truly care and he has received the treatment and therapy that was needed. Dad will be discharged soon and it’s because of the great care provided by his nurse CJ and Dr Sharobian.  I’m truly thankful for everyone at Community Care & Rehab Center.

Community Care Rehab has friendly service and amazing staff. The call lights are answered quickly, the nurses are responsive and the food is delicious. Your family member will be well Taken care of and very happy at this beautiful facility! My favorite part of Community Care is their beautiful garden patios throughout the building and the specialized occupation therapy room.

I had a great experience with Community Care & Rehab. From the marketer Rachael to all the staff inside the facility. They were on top of my loved one always making sure she had everything she needed. They always answered our questions and were very supportive of me and my family! I will continue to refer to them going forward to my other family and friends.

We were looking for a reputable custodial facility that can take my dad who’s a Medi-cal patient.  Thankfully, I was able to find Community Care & Rehab by luck.  I mostly worked with Carlos and he was so nice and accommodating with the whole process of getting my dad placed in there. It took us awhile to get him in due to some paperwork delays but they were very patient with us. They were always very responsive whenever we had questions.  The employees there are also very nice and accommodating.   They also follow appropriate guidelines to ensure that the patients are safe.  They tested my dad for COVID and TB to make sure that he’s okay to stay there.  I’m happy that we found a place that will take care of my dad as he ages.
